Birth and Death Certificates

Congratulations on the birth of your new baby!

To register your baby please submit the birth registration information to the Government of Ontario through Service Ontario Register a Birth within 30 days of the birth. In addition, the Newborn Registration Service with Service Ontario also allows you to apply for a Birth Certificate and Social Insurance Number all at once.

Death Registrations & Certificates

In the event of a death, the Funeral Director provides a Statement of Death and a Medical Certificate of Death to the municipality in order to obtain a permit for the purposes of burial or cremation.  The information received from the Funeral Director is recorded in the municipality’s database, and the original paperwork is forwarded to the Provincial Office of the Registrar General for registration.
